Gabriele (angus?) forgot to do a (cd htdoc; perl cf_generate.pl) before
committing his changes to htcommon/defaults.cc, so cf_byname.html is
missing tcp_max_retries and tcp_wait_time, both of which should go in
the server block.

I haven't taken a thorough look at all of these, but they look reasonable
to me. The questionable ones can probably stay where you put them -
certainly one user would be grateful for an URL-dependent bad_word_list
attribute. I'm not sure about date_format and iso_8601 - they're only
used in htsearch, aren't they? What do server block attributes mean
to htsearch?
